Welcome to the Step Up to Good Health Flu-Free Zone website. This website will provide important information about preventing the spread of influenza. You will be able to review flu health screening forms and vaccine information statements (VIS) before receiving the vaccine. If you are not able to accept the flu vaccine, you can decline vaccination here. If you are declining vaccination for any reason (including being vaccinated elsewhere), in addition to completing the declination form, you DO need to provide documentation for the declination reason by November 7, 2013. After you submit the declination form, you can click on the link to print the form to be completed for medical or religious declination. You will be considered non-compliant until documentation is received, reviewed and approved. Please scan and upload your documentation to the website or fax to 410-630-7460. Vaccination declination information will be transferred to the Employee Health database. Your manager will be able to see in KBMI that you complied with the Influenza Policy. The deadline for flu compliance is 12/5/13.
As part of our Step Up to Good Health Program, we are encouraging all employees to be vaccinated. The flu is a serious and contagious respiratory illness caused by influenza viruses. It can cause mild to severe illness and in some cases can lead to death. The best way to prevent the flu is with frequent hand washing and by getting a flu vaccination each year. This is especially important for health care workers. We need to protect ourselves, our families and our patients. New this year, an egg free vaccine for employees who are < 49 years old will be available in the Employee Health Office - EGG FREE VACCINE AVAILABLE IN NOVEMBER.
